Abstract: Odontogenickeratocyst is infrequent and benign ,but locally aggressive neoplasm. Highly varied clinical presentation makes its clinical diagnosis difficult. This is a casereport of rare presentation ofodontogenickeratocyst with large extraoral swelling and multiple sinus openings which resemble osteomyelitis clinically. Timely diagnosis and accurate treatment plan is mandatory for better prognosis of patient and reduces the risk of recurrence
